The
two supervision hours documentation forms are downloadable by clicking
on the pictures of Sigmund and Albert to
the right. By
clicking on Sigmund Freud, you may download Supervision Form B. Albert
Ellis will yield Supervision Form A. They were created using
Corel WordPerfect using the Tables
feature. I then published them in PDF format. I
modified them recently using OpenOffice. You could
design
similar forms using the Tables feature of any Word Processor
software. If you do not have
Adobe
Reader, here is a link so you can
download it. I no
longer use WordPerfect. Lately, I actually have begun to use Google
Documents www.google.com/documents.
I
began using Form A at the start of my Post-Doc, and ended up using Form
B, which I actually preferred.
The forms I am sharing represent a few hours work. Sending them out and creating this webpage to distribute them required a few hours work. If you use my forms and you find benefit from them in any way, I would like to ask that you make a donation to the OpenOffice.org. I think a donation of $5-$7 is appropriate.
